Therapy West, Inc. & pLAy Studio is contracted with various insurance companies, regional centers, and school districts. Therapy services can be funded in the following ways:
The following is a list of frequently asked questions and general policies regarding the use of insurance for therapy services.
We accept various PPO insurance. Your health insurance card (copy of front and back) is needed to verify eligibility.
We will need a copy of your health insurance card (front and back), your child’s date of birth, and a prescription for requested services with medical diagnosis to determine eligibility and benefits.
All payments are due at the time of service. We accept credit card (Mastercard, Visa, Discover), check, and cash.
Unfortunately, prior authorization does not guarantee payment. Payment is determined at the point of claim (time at which your insurance company receives and processes the claim for provided services). Payment in full is due at the time of service.
You are responsible for any deductible or co-pay according to your insurance plan. If Therapy West, Inc. is a preferred provider and your insurance denies coverage or payment for the provided service, you are responsible for the full amount.
